Our application process helps us determine if our program is the right fit based on independence, safety, and community readiness.
How to Apply
- Complete the Pre-Intake Application Form
(This helps us determine eligibility and availability.)
- We contact you within 24–48 hours
A team member will review your application and reach out to discuss next steps.
- Schedule a House Tour + Intake Interview
If approved during pre-screening, we will schedule a tour (in-person or virtual).
- Placement decision + Move-In Coordination
If accepted into the program, we will provide a move-in checklist and next steps.
Move In Requirements

- Valid ID
- Proof of income (SSI, SSDI, private pay, etc.)
- First Month Program Fee + Community Fee

We do not provide personal care, medical care, or assisted living services.
